What documents are required to open a file?

A medical referral is required. It must be addressed to psychiatry (or directly to Autisme M.D. Clinic) and specify that it is a request for an autism spectrum disorder (ASD) assessment.

How do I submit my medical referral?

Check our homepage to see if the waitlist is open for the patient's age group.
If it is, click the Submit a New Assessment Request button and attach your medical referral to the admission form.

Question 3

What happens once my file is accepted?

Accepted Answer

If the medical referral is valid, we will open your file and contact you to add you to our waitlist. You will receive a confirmation of your registration by email.

Question 4

Do I need to complete any documents before the first appointment?

Accepted Answer

No. If you have filled out the intake form on our website and submitted your valid medical request, you don't need to fill out any other documents. Please submit any documents relevant to the evaluation on our Add Documents page.
