New Patients
We are once again accepting new assessment requests for patients UNDER 18 YEARS OLD, with a valid medical referral.

For the referral to be valid—and for the assessment to be covered by the RAMQ—it must be addressed to Autisme M.D. Clinic or simply to psychiatry (without specifying a location), and must include the note “ASD assessment”.

Any referral directed to psychology, neuropsychology, or any discipline other than psychiatry will be refused. A new medical referral will be required for the assessment request to be accepted.

Only requests submitted through this form will be accepted. Assessment requests sent by email will not be considered.

Waitlist Information
Once we have received your assessment request, we will contact you to explain the steps to complete the addition of your child to our waitlist.
We will also reach out again when it is time to schedule the appointment.

If we do not receive a response after three contact attempts, the file may be closed.
